Thank you for choosing BVA Advanced Eye Care! We understand how difficult it is to verify insurance coverage while also determining your level of personal responsibility. Medical and routine eye care service involves verification of your health insurance when applicable, and we would be happy to verify your insurance coverage for you. To do so, be sure to provide our staff with the following things prior to your appointment:
- Name, date of birth and ID# on the card
- Name of your insurance
- Group name if applicable
- Customer Service phone number usually located on the back of the card
Though we are willing to verify your insurance coverage for you, we do so as a courtesy. Ultimately, you are responsible for any remaining balance that your insurance denies or deems as a non-covered service. To ensure that you receive the proper coverage, please call your insurance company or refer to your insurance handbook.
