Patient Registration

If you are a new patient and have scheduled an appointment, you will need to complete the online registration before your appointment. If you have questions or concerns please give us a call, we are here to help.

Things to Remember for Your Appointment

Arrive 10-15 minutes early to allow time for the check-in process. Bring the following items with you:

  • Photo Identification
  • Insurance Card (medical and vision, if applicable)
  • Any relevant medical records for existing conditions related to eye care
  • Fill out all online forms prior to your appointment
  • Bring a list of any and all medications and allergies
  • Check with the office if you are not sure about removing contact lenses prior to your appointment
    Check with the office to find out if you need a driver

Please note: Though we try to adhere to your scheduled appointment time, emergency “work-ins” or unforeseen problems may occur and cause your doctor to run behind schedule. If the front desk is aware of any such circumstances, you will be notified at the appropriate time.

Insurance

We understand how difficult it is to verify insurance coverage while also determining your level of personal responsibility. Medical and routine eye care service involves verification of your health insurance when applicable, and we would be happy to verify your insurance coverage for you. To do so, be sure to provide our staff with the following things prior to your appointment:

  • Name, date of birth and ID# on the card
  • Name of your insurance
  • Group name if applicable
  • Customer Service phone number: usually located on the back of the card

Though we are willing to verify your insurance coverage for you, we do so as a courtesy. Ultimately, you are responsible for any remaining balance that your insurance denies or deems as a non-covered service. To ensure that you receive the proper coverage, please call your insurance company or refer to your insurance handbook.

Financing Options

Correcting your vision is one of the best investments you can make. Here at BVA advanced eye care, we understand investing in advanced eye care is a big decision. We want to work with you to create a financial plan!

BVA offers several payment options, and you can even pay online  

  • Cash or Check
  • Credit Card: American Express, Discover, Master Card and Visa
  • CareCredit
  • Flex Spending Accounts and Health Savings Accounts

We strongly encourage you to talk with us to go over the benefits of correcting your vision. For some patients, you may end up saving money in the long run!

Do you wear contacts? you may save money over time. Let’s assume you spend $25 for a box of contacts – on average you will need about 10 boxes a year = $250 per year just for contacts. Now, if you have to buy more complex contacts (i.g. toric contacts) you are likely to be spending around $500 to $700 per year.

BVA does not want money to be your only reason to not correct your vision. Check if your place of work offers a flex spending account or health savings account because you can use these benefits towards correcting your vision.  

 

What Is Care Credit?

We are happy to accept CareCredit, a health care financing program that offers monthly payments.

CareCredit will work with you to design a monthly payment plan that fits your budget. Once you have been accepted by CareCredit, we are able to begin your steps to better vision ahead!
